I don't think any of the steps that you described included transferring the
system over to the new harddrive.

eg. c:\sys d:

This should be done after the format step (if not done as part of the
formatting), prior to the cloning run. I don't think the fdisk /mbr step is
necessary because it simply re-builds the master boot record if it becomes
corrupted (and should not be used under some circumstances, such as when
Bootmagic is being used for dual boots).
